One man is dead after a fatality accident Thursday morning in Mineola. The one-vehicle crash occurred around 11 a.m. when the silver Lexus left the roadway heading westbound on McDonald Street and landed in an embankment. 54-year-old Jay Forrest Corley was pronounced dead at the scene. No passengers were in the vehicle. The crash is under investigation.
